Application must be mailed to new eyes with prescription for processing, emails and faxes are not accepted. For school age children, the school nurse can write a letter on official school letter head and submit that instead of an application.
Description
Provides vouchers for individuals to exchange for glasses at participating locations. Application must be mailed in with a copy of an eye glass prescription by a social worker, church, primary care physician or medical clinic. An eye doctor can not make the referral.

Mon-Thu 9:00 am-4:00 pm, closed for lunch 12:30 pm-1:30 pm (EST)
