Description
Provides free medical air and ground transportation for children or adult patients and an escort needing transportation for medical evaluation, diagnosis or treatment with a destination within 1,000 miles. Accepts requests from doctors, nurses, patients, social workers, transportation coordinators and family members. A minimum of 5 business days are required to coordinate a flight.

Eligibility
Must have/meet: * Financial need for assistance * Medically cleared to fly in a non-pressurized small aircraft * Ambulatory and sit upright in a standard aircraft seat * Not require medical care en route * Not have a communicable disease * If supplemental patient oxygen is required, it must be provided by the patient and be in a Department of Transportation pressure test approved bottle * Provide own ground transportation to and from airport as well as be responsible for lodging arrangements
