A1 Case Management

Description

Provides case management for children and adults with developmental disabilities that focus on enabling the individual to attain maximum functional level. Services are provided to participants in the Kentucky Medicaid waiver programs including the Supports for Community Living and Michelle P. Waivers.

Mon-Fri 7:30 am-3:30 pm (EST)

Required Documents

Please bring disability documentation or psychiatric evaluation with you, if not readily available will work with family to obtain these

Eligibility

Currently enrolled or meet requirements for Supports for Community Living or Michelle P. Waivers or have a documented IQ of 70 or below
