Description
Offers help to eligible individuals (including older adults, people with disabilities and people with low incomes) prepare their tax returns or complete paperwork to apply for tax relief in the form of tax credit, rebate, exemption or postponement programs.

Please bring with you: * Government-issued picture ID for each taxpayer on the return * Social Security cards or Individual Taxpayer Identification Number (ITIN) documentation for all taxpayer(s) and dependents on the return * Copy of previous year's tax return(s) * Income documents for wages (W-2 forms), self-employment or rental income, interest (1099-INT), dividends (1099-DIV), capital gains/losses, unemployment compensation (1099-G form), prizes, gambling winnings (Form W-2G), awards, pensions and other retirement income (1099-R), Social Security benefits (SSA-1099) * Any form 1099 documents, including form 1099-MISC, 1099-K, and 1099-Q * Brokerage statements - sale of stocks or bonds * Forms 1095-A, B or C, Health Coverage Statements * If itemizing deductions: records for mortgage interest (Form 1098), medical/dental expenses, charitable donations; sales, income or property taxes * Records of federal and state income taxes paid (including quarterly estimated tax payments) if not shown on income documents * Total amount paid for daycare or other dependent care provider and the provider's information - name, address, telephone number, and their Social Security number or business Employer Identification Number * Proof of bank account routing number and account number, such as a blank check, if taxpayer wants direct deposit of refund or direct debit of balance due * Any other recent IRS or state tax department correspondence
